Agency Questionaire

Thank you for your interest in becoming a Food Bank partner agency. The following questionnaire (click here) will assist you in the requirements for partnership. If you have questions about partnership, please contact:

What type of program does your agency operate? (A description of types of programs is listed below.)

Soup Kitchen
Homeless Shelters
Food Pantry
After School Care
Youth Program
Day Care Center
Senior Center
Shelter
Rehabilition Center (Substance Abuse or Disability)

Food Pantry	A program providing bags or groceries to individuals and families for home preparation and consumption.
Soup Kitchen	A program that provides a prepared meal, generally in congregate eating setting. An individual(s) in the agency must have ServSafe certification.
Senior Center	A program providing services to the elderly (age 60 and over).
Shelter	A program that provides temporary shelter to needy individuals.
Day Care	A program that provides on site childcare to children up to 6 years of age.
Group Home, Rehab-Disability	A program that provides services to individuals who are developmentally and physically challenged.
Rehab-Substance Abuse	A program that provides alcohol and substance abuse treatment and services to individuals.
Youth Services	A program providing food and activities to children between the ages of 6 and 18.

Soup Kitchen, Homeless Shelter or Food Pantry: Your program must serve a minimum of 50 individuals on a monthly basis. Do you meet the requirement? Based on your answer, your program is not qualified for partnership at this time. Your agency must serve a minimum of 50 different individuals on a monthly basis to meet the requirement.
After School or Youth Program, Day Care Center, Senior Center, Group Home, Rehabilition Center (Substance Abuse or Disability): Group Homes must be licensed for at least 6 individuals. Second Harvest can not do inspections inside residences. Do you meet the requirement? Based on your answer, your program is not qualified for partnership at this time. Your agency must be licensed to serve 6 individuals. Second Harvest can not inspect individual residences due to policies and procedures of America’s Second Harvest.
Does your food program provide food free to all clients? Based on your answer, your program is not qualified for partnership at this time. Your agency must serve food free to at least 90% identifiable low-income clients. Food Bank products must not be sold or bartered. To do is immediate grounds for suspension from the Food Bank.
Does your community food program serve the poor, ill, and needy or homeless? Based on your answer, your program is not qualified for partnership at this time. Your agency must serve at least 90% identifiable low-income individuals among its clients.
Is your agency a tax-exempt organization as describe in Section 501 (c ) 3 of the Internal Revenue Code of the U.S. Treasury Department? Based on your answer to the survey, your program is not eligible for partnership. You agency must obtain and maintain at 501 c 3 nonprofit status from the IRS to qualify for partnership. To apply for a 501 c 3 status, please visit the IRS website at: http://www.irs.gov/pub/irs-pdf/k1023.pdf.
Does your feeding program have a record keeping system in place such as names and/or number served? Based on your answer to the survey, your program is not eligible for partnership. Second Harvest requires that records be kept on all individuals and households served to ensure that Food Bank product goes only to eligible recipients. Emergency Pantries must show at a minimum of client’s name, address, phone number, the number in the family and the date served by your agency.
Is your program able to utilize the Food Bank at least once every 3 months from the Food Bank? Based on your answer to the survey, your program is not eligible for partnership. Second Harvest requires partner agencies to shop a minimum of once on a 3 month basis.
Does your program agree to provide the Food Bank with the formal listing individuals authorized to take full responsibility of the program’s services (List of Board Members or governing authority for your program)? Based on your answer to the survey, your program is not eligible for partnership. Second Harvest requires a roster of the governing board of your organization, giving name, address, telephone number and term of membership. Follow this link for Criteria for Agency Boards.
Is your program able and willing to submit completed reports on a quarterly basis? Based on your answer to the survey, your program is not eligible for partnership. Second Harvest is required to submit quarterly reports to America’s Second Harvest. Your agency must submit reports in timely manner on the individuals and households you are serving in a given month. Failure to complete this report could result in suspension and/or termination.
Is it mandatory for clients to participate in religious services in order to receive food? Based on your answer to the survey, your program is not eligible for partnership. Your agency cannot exclude people from its services based on race, creed, national origin, religious affiliation, sexual preference, sex, age, or handicap.
Will your agency allow a food bank representative to visit your program as needed (at least every 2 years)? Based on your answer to the survey, your program is not eligible for partnership. Second Harvest is required to visit your agency at least every two years to ensure that donated foods and being used and stored properly. During this visit a representative will request updated information and renew your partnership agreement with Second Harvest.
Does your feeding program advertise to the community of your services? Based on your answer to the survey, your program is not eligible for partnership. Your feeding service must be regularly scheduled, operating at least, but no limited to twice a month. Exceptions are allowed for school-based organizations.
Does your feeding program operate out of a personal residence? Based on your answer to the survey, your program is not eligible for partnership. Second Harvest does not permit an agency to operate out of a personal residence.
Does your agency share food storage space with programs from other agencies? Based on your answer to the survey, your program is not eligible for partnership. Your agency can not share food with an agency that is not a Second Harvest approved agency. Failure to comply with this policy could result in suspension and/or termination.

	Based on your answers you have provided, your program meets the initial eligibility requirements for being considered for partnership. As the largest hunger relief charity in the Metrolina area, Second Harvest Food Bank of Metrolina works to end hunger. We do this by supplying a network of over 600 non-profit agencies and churches in 19 counties with food and essential grocery products.
